What you will do:

- Conduct thorough assessments to develop effective diet plans for community patients, with a focus on long-term conditions such as type 2 diabetes and obesity.
- Provide expert nutritional advice and tailor educational programs to meet the diverse needs of patients.
- Collaborate with multidisciplinary teams to ensure cohesive and comprehensive care for patients within the community.

- Assist in outpatient clinics, bringing your skills to the forefront to improve patient outcomes.

*To excel in this role, you need at least three years of experience as a dietitian, with a background in outpatient and community settings. Proficiency in managing long-term conditions, specifically type 2 diabetes and obesity, is essential. You must be able to work independently with minimal support and be HCPC registered. Additionally, a driving licence is essential as you will be required to travel within the community.
