What you will do:

  • Conduct comprehensive Care Act assessments to ensure individuals' needs are met and independence promoted.
  • Engage with hospital discharge pathways, contributing valuable insights at MDT meetings both on the wards and within the community.
  • Collaborate effectively with ward staff, healthcare providers, and social care professionals to coordinate seamless, person-centred discharges.
  • Support the Discharge to Assess (D2A) model by completing assessments in community settings or bed-based placements.
  • Partner with the Reablement Team to facilitate timely access to necessary services post-discharge.
  • Address safeguarding concerns as needed, conducting enquiries under Section 42 of the Care Act.
  • Maintain meticulous case records and documentation in adherence to local authority standards.
  • Participate in team duty functions, sharing knowledge and contributing to the team's collective expertise.
  • Champion a strengths-based approach, ensuring alignment with person-centred values and current legislation.
