What you will do:
- Provide day-to-day leadership and support to a team of care and support staff within the community living service.
- Drive the promotion of person-centred care, ensuring that the individual needs, preferences, and goals of service users are at the core of care planning and delivery.
- Lead on the formulation and review of individual care and support plans, ensuring they are tailored to each service user's requirements.
- Create and maintain a safe, nurturing, and empowering environment for service users.
- Collaborate effectively with families, social workers, allied health professionals, and external agencies to ensure continuity of care and the best possible outcomes for service users.
- Uphold and ensure compliance with safeguarding policies and procedures.
- Participate in staff supervision, performance reviews, and contribute to training and development programs.
- Maintain accurate records and ensure all documentation complies with organisational and regulatory standards.
- Assist in rota planning and effective resource management, ensuring the team can deliver high-quality care.
Requirements:
- Essential — NVQ Level 3 in Health and Social Care (or equivalent).
- Essential — Previous experience in a care or support role, preferably with adults with learning disabilities.
- Essential — Proven leadership skills with the ability to effectively supervise and inspire staff.
- Strong communication and interpersonal abilities.
- A sound understanding of the principles of community-based support and person-centred care.
- A flexible, proactive approach and a commitment to delivering high-quality care.
