What you will do:

• Provide expert reablement occupational therapy following hospital discharge to help patients regain their independence.
• Conduct Care Act Assessments, Mental Capacity Assessments, and facilitate Best Interest Decisions ensuring comprehensive care is delivered.
• Apply your current moving and handling training to assist patients safely and effectively.
• Review bed levels and implement bed rails ensuring patient safety and comfort.
• Work collaboratively with a dynamic team contributing to the overall wellbeing of the community.

To qualify for this role, you must be HCPC registered and have a valid driving licence. These are essential to ensure the delivery of high-quality and accessible care.
