What You Will Do:

Patient Assessment: Independently triage and assess patients to ensure timely, safe, and efficient discharges, addressing both clinical and emotional needs.

Home Visits: Conduct home visits to facilitate smooth transitions for patients moving from hospital back to their home environment.

Equipment Provision: Provide necessary equipment to support patient recovery, ensuring they can regain independence at home.

Collaboration: Work closely with the multidisciplinary team, including healthcare professionals, to ensure seamless discharge planning and patient care.

Utilise Expertise: Apply your NHS experience and post-qualification skills in an IDH (Integrated Discharge Hub) or Discharge to Assess model.

Travel: Use your own car (with access to transport) to cover multiple locations, enhancing the reach and efficiency of the service.

Maintain HCPC Registration: Ensure continuous professional development and uphold the standards required by the Health and Care Professions Council (HCPC).
