What you will do:
- Conduct person-centred Care Act assessments and mental capacity assessments within hospital environments.
- Facilitate and coordinate discharge planning using Discharge to Assess (D2A) models to promote smooth transitions for patients.
- Collaborate closely with health professionals, community services, and families to develop appropriate care and support packages.
- Assist adults with complex needs, ensuring their safety and well-being through necessary safeguarding interventions.
- Maintain detailed and accurate case records and reports to reflect the care provided and outcomes achieved.
- Adhere to relevant legislation and council policies, championing best practices and ethical conduct.
Requirements:
- A Social Work qualification (BA/MA Social Work, DipSW, CQSW or equivalent).
- Current registration with Social Work England.
- Proven experience in hospital discharge planning or within an adult social care setting.
- Strong understanding of legislation such as the Care Act 2014 and Mental Capacity Act 2005.
- Proficiency in managing a caseload efficiently under pressure.
- Demonstrable skills in communication, assessment, and report writing.
