What you will do:

• Assess and evaluate the needs of adults in the community, recommending appropriate interventions and equipment to enhance their independence and quality of life.
• Work as part of either the Adults Community OT team or the Home Adaptations Team, attending to waiting lists and high-priority cases efficiently and compassionately.
• Collaborate with a multidisciplinary team to develop comprehensive care plans and ensure the seamless delivery of support services.
• Provide expert guidance on home adaptations, improving accessibility and safety for individuals living at home.
• Ensure documentation and reports are maintained to the highest standards, reflecting the best practices in occupational therapy.
• Be an advocate for patients, driving improvements in service delivery in line with organisational strategies and goals.

To be successful, you need to have HCPC registration and be a confident car driver to reach our clients throughout the community.
