This is an ongoing full-time locum position for a highly skilled Social Worker Level 2 to join our dynamic Adults 18+ Team within the Adult Care and Community Wellbeing service. What you will do:
- Manage a varied and complex caseload, supported by the Lead Practitioner, ensuring high standards of professional practice.
- Conduct comprehensive needs assessments in line with the Care Act 2014, employing a strengths-based approach to identify solutions and promote independence.
- Engage with specialist safeguarding colleagues to conduct safeguarding enquiries, keeping the person at the centre of all responses.
- Serve as a deputy for the Lead Practitioner, including attending multi-disciplinary meetings.
- Supervise and mentor non-registered social care practitioners and support the development of students and newly qualified social workers.
- Collaborate with partners to address broader needs beyond social care, such as health and housing.
- Create cost-effective care packages in collaboration with individuals, carers, and agencies, ensuring quality of life.
- Guide individuals, families, and communities towards informed choices about their needs and support available.
- Map available resources and identify service gaps.
- Maintain and update case notes, and prepare reports, providing evidence in court if required.
- Work in compliance with Lincolnshire County Council Adult Care policies and procedures, including safeguarding.
