Key Responsibilities:

• Social Care Needs Assessments: Carry out detailed assessments for individuals and carers in line with the Care Act 2015 and national eligibility criteria.

• Support Plan Development: Collaborate with service users and carers to develop personalised support plans aimed at promoting independent living.

• Safeguarding and Statutory Duties: Perform statutory safeguarding responsibilities, conduct Mental Capacity Act assessments, and support hospital discharges for individuals under secondary mental health care.

• Interagency Collaboration: Build and maintain effective partnerships with external agencies and community services to provide holistic care.

• Review and Reassessment: Regularly review and reassess care plans and support packages, promoting the use of personal budgets to meet individual outcomes.

• Direct Support Services: Offer counselling, guidance, and practical support to service users and their carers, helping them live fulfilled and independent lives.

Essential Skills and Qualifications:

• Qualified Social Worker registered with Social Work England.

• Minimum of 5 years’ post-qualification experience in adult social care within statutory settings.

• Strong understanding of mental health legislation, Care Act 2014, and safeguarding processes.

• A full, clean driving licence and the ability to travel within the borough as required.
