Role Purpose:

As a Senior Adult Social Worker, you will lead and deliver exceptional social work practice within the Review Team. You will focus on reviewing and enhancing care and support plans, ensuring they meet the changing needs of service users while prioritising safeguarding and independent living.


What You Will Do:

Assessment and Care Review:

  • Conduct comprehensive reviews of care and support plans, adhering to the Care Act 2014.
  • Evaluate and update care packages to ensure they remain responsive and effective.

Case Management:

  • Manage a complex caseload independently, maintaining accurate records that meet legal and policy standards.
  • Identify and investigate safeguarding concerns, participating in multi-agency safeguarding enquiries.

Collaboration and Partnership:

  • Work closely with health professionals, community organizations, and local agencies to deliver integrated support.
  • Engage service users, carers, and families, ensuring their voices shape care decisions.

Leadership and Development:

  • Mentor and supervise junior staff and social work students, fostering their professional growth.
  • Contribute to service improvements, helping implement best practices across the team.

Requirements:

Essential:

  • Degree in Social Work or equivalent qualification.
  • Registered with Social Work England.
  • Proven post-qualification experience in adult social care, with expertise in care reviews.
  • Strong understanding of the Care Act 2014, Mental Capacity Act 2005, and safeguarding protocols.
  • Exceptional analytical, decision-making, and communication skills.
  • Ability to independently manage a complex caseload effectively.

Desirable:

  • Previous experience within a review team or similar environment.
  • Practice Educator qualification or equivalent supervisory experience.
