Role Purpose:

To deliver high-quality social work practice within the Mental Health Team, supporting adults with complex mental health needs. This includes assessments, care planning, and interventions to promote recovery, independence, and safeguarding.


What You Will Do:

Assessment and Intervention:

  • Conduct comprehensive assessments of adults with mental health needs in line with the Care Act 2014 and Mental Health Act 1983 (as amended).
  • Develop and implement individualized care and support plans to promote mental well-being and recovery.
  • Undertake mental capacity assessments and support best interest decision-making when required.

Case Management:

  • Manage a complex caseload of adults with mental health conditions, ensuring timely interventions and effective service delivery.
  • Maintain accurate case records and documentation in compliance with statutory guidelines and local policies.
  • Monitor and review care plans regularly, ensuring they remain responsive to changing needs.

Safeguarding:

  • Identify, investigate, and respond to safeguarding concerns, ensuring the implementation of appropriate protection measures.
  • Participate in multi-agency safeguarding processes and contribute to safeguarding enquiries.

Collaboration and Partnership:

  • Work closely with health professionals, housing services, voluntary organisations, and other agencies to deliver integrated mental health support.
  • Engage service users, carers, and families, ensuring their views shape care planning and decision-making.

Leadership and Development:

  • Provide mentorship, support, and guidance to junior staff and social work students.
  • Participate in supervision, reflective practice, and continuous professional development.
  • Contribute to service improvement initiatives and best practice development within the Mental Health Team.

Requirements:

Essential:

  • Degree in Social Work (or equivalent qualification).
  • Registered with Social Work England.
  • Significant post-qualification experience working in adult mental health services.
  • Strong knowledge of the Care Act 2014, Mental Health Act 1983, and Mental Capacity Act 2005.
  • Excellent assessment, decision-making, and communication skills.
  • Ability to independently manage a complex caseload effectively.

Desirable:

  • Approved Mental Health Professional (AMHP) qualification or willingness to undertake AMHP training.
  • Experience in integrated health and social care settings.
