What you will do:

- Deliver high-quality care for patients under the Discharge to Assess pathway, focusing on those within the Non-Weight Bearing Pathway.
- Conduct thorough assessments to evaluate patients' rehabilitation needs and develop personalized care plans.
- Work in tandem with local authority social services and continuing healthcare teams to support onward assessment and discharge planning effectively.
- Engage proactively with the Proactive Assessment Unit in Folkestone, championing patient-based strategies and successful discharge outcomes.
- Facilitate safe, efficient discharges while ensuring continuity of care post-hospital stay.

Requirements:

- HCPC Registration is essential.
- Valid driver's licence and access to a car for necessary travel.
