What you will do:

- Independently assess, treat, and plan discharges for patients as part of the community rehab team.
- Engage in equipment reasoning and ordering, ensuring the right resources are provided.
- Utilise recognisable outcome measures to track patient progress.
- Conduct family meetings to discuss patient care and progress.
- Utilise your expertise and experience within a community and rehab setting to drive positive outcomes for patients.
- As this role involves community work, being a car driver is preferable to enhance mobility and service delivery.

Prerequisites for the role include being HCPC registered and ideally having post-qualification experience in the community and rehab setting.
