What you will do:

• Undertake Care Act assessments and reviews for young people transitioning into adult services.
• Collaborate with children’s services, education, health professionals, and families to develop effective transition plans.
• Provide advice and guidance to young people and their families about adult services, rights, and options.
• Develop and implement personalised care and support plans based on identified needs and outcomes.
• Promote independence, choice, and wellbeing, ensuring that the voice of the young person is central to all decision-making.
• Work in accordance with legal and statutory requirements, including the Care Act 2014, Children and Families Act 2014, and Mental Capacity Act 2005.
• Maintain accurate and timely case records and reports in line with service procedures.
