We have an exciting opportunity for a Band 6 Occupational Therapist specialising in Discharge to Assess, located in the vibrant city of Birmingham. This full-time role, offering a competitive pay rate of £26 hourly, requires you to make a difference from 8am to 4pm, Monday to Friday. What you will do:

• Conduct comprehensive assessments and deliver treatment and discharge planning specifically for complex and frail elderly patients.
• Engage in the Discharge to Assess process using your expertise to ensure smooth transitions from inpatient settings.
• Manage and prioritise a caseload, ensuring tailored interventions to meet individual patient needs.
• As a car driver, perform Assess visits as required to support patients in their transition to home environments.
• Utilise the RIO system effectively to document and communicate patient information if familiar.

To excel in this role, you need to be HCPC registered, which ensures you maintain high standards of practice in your professional work.
