What you will do:
- Intervene with residents who can be supported home from hospital emergency departments to avoid admissions.
- Manage a caseload of 5-7 cases, ensuring each is progressed efficiently within a 10-day timeframe.
- Collaborate with the multi-disciplinary team to stabilise residents, implementing urgent care plans and transferring cases to Reablement.
- Complete Care Act Assessments, develop Care and Support Plans, and review existing care arrangements.
- Engage in safeguarding, mental capacity assessments, and Best Interests Decision Making.
Requirements:
- SW qualified and registered, with at least 2 years post-qualifying experience(post ASYE).
- Experience in fast-paced services like Urgent Response or hospital discharge is desirable.
- Understanding of Reablement ethos and strengths-based practice is an advantage.
