Working Hours: Full-Time (37.5 hours per week; Monday to Friday, 9am–5pm with flexible working options)

Main Responsibilities

  • Manage a caseload of clients, collaboratively developing care plans, safety plans, and risk assessments.

  • Consult with and refer to other disciplines or external agencies as needed, in partnership with the multidisciplinary team.

  • Respect service users’ dignity, beliefs, and preferences throughout assessments and care planning.

  • Identify and manage risk to individuals and, where applicable, to their families.

  • Create and implement individualised crisis plans that reflect the needs and preferences of service users.

  • Ensure compliance with all relevant legislation, policies, and procedures, highlighting unmet needs or resource gaps.

  • Apply the Mental Health Act as appropriate, in line with the Code of Practice and trust policies.
