Main Responsibilities

Manage a caseload of clients, collaboratively developing care plans, safety plans, and risk assessments.

Consult with and refer to other disciplines or external agencies as needed, in partnership with the multidisciplinary team.

Respect service users’ dignity, beliefs, and preferences throughout assessments and care planning.

Identify and manage risk to individuals and, where applicable, to their families.

Create and implement individualised crisis plans that reflect the needs and preferences of service users.

Ensure compliance with all relevant legislation, policies, and procedures, highlighting unmet needs or resource gaps.

Apply the Mental Health Act as appropriate, in line with the Code of Practice and trust policies.
