What you will do:

• Review and assess incoming referrals and contacts within the MASH to determine appropriate pathways and responses.
• Conduct threshold conversations and home visits to engage children, families, and carers, ensuring their safety and support.
• Collaborate closely with team members, including social workers, health professionals, education specialists, and police officers.
• Develop and maintain comprehensive records, reports, and documentation to facilitate case management.
• Provide informed recommendations and case presentations to management teams for effective service delivery.
• Participate in multi-disciplinary meetings, interfacing with various agencies to coordinate efforts and enhance service outcomes.
