Job Summary:
The post-holder will work as a key member of the Community Mental Health Team (CMHT), delivering high-quality mental health care to individuals with complex mental health needs within the community. They will provide comprehensive assessment, care planning, and evidence-based interventions, while supporting service users to achieve their recovery goals. This role requires flexibility, strong clinical judgement, and the ability to work autonomously as well as part of a multi-disciplinary team.

What you will do:

  • Undertake the role of Care Co-ordinator for a caseload of service users with severe and enduring mental illness, in accordance with the Mental Health (Wales) Measure and Care and Treatment Planning guidance.
  • Complete holistic assessments, risk assessments, and collaborative care plans in line with local and national policy.
  • Administer and monitor prescribed medication, including depot injections and Clozapine, adhering to relevant protocols.
  • Work closely with service users, families, carers, and other agencies to promote recovery, social inclusion, and wellbeing.
  • Engage in multi-disciplinary team meetings, case conferences, and reviews, contributing to shared decision-making.
  • Support the delivery of crisis intervention and risk management, including out-of-hours duties where applicable.
  • Maintain accurate, up-to-date clinical records and documentation in accordance with NMC and organisational standards.
  • Provide mentorship, supervision, and support to junior staff, students, and unregistered team members as required.
  • Participate in audit, clinical governance, and service development initiatives to ensure continuous quality improvement.
