What you will do:
- Conduct comprehensive manual handling assessments and provide tailored interventions for citizens with complex needs.
- Collaborate with a range of professionals in home visits within the Birmingham area, ensuring exceptional service delivery.
- Create and implement effective rehabilitation programs to enhance citizens’ daily living activities.
- Facilitate safe and efficient discharge from hospital settings and ensure seamless transition into home environments.
- Maintain detailed case records and documentation in accordance with HCPC standards.
The post holder must be a registered Occupational Therapist with the Health and Care Professions Council (HCPC) and hold a valid UK driving licence with access to a vehicle for work purposes.
