What you will do:

• Undertake Care Act 2014 assessments and reassessments.
• Complete strengths-based assessments to identify eligible needs and outcomes.
• Manage a complex and varied caseload of adults with care and support needs.
• Develop, implement, and review person-centred support plans.
• Promote independence, wellbeing, choice, and control for service users.
• Conduct risk assessments and develop risk management plans.
• Undertake Mental Capacity Act (MCA) assessments.
• Facilitate and participate in Best Interests meetings and decisions.
• Ensure compliance with the Mental Capacity Act 2005 and related legislation.
• Complete safeguarding enquiries under Section 42 of the Care Act.
• Investigate concerns relating to abuse, neglect, self-neglect, and exploitation.
• Develop and monitor safeguarding plans to reduce risk and promote safety.
• Carry out carers' assessments and identify support needs for unpaid carers.
• Provide advice, guidance, information, and signposting to individuals and families.
• Arrange, coordinate, and review care and support packages.
• Liaise with residential, nursing, domiciliary, and community care providers.
• Work collaboratively with health professionals, housing services, and voluntary organisations.
• Attend multidisciplinary team meetings and case conferences.
• Participate in safeguarding strategy meetings and review meetings.
• Support hospital discharge planning and ensure safe transitions into the community.
• Prevent unnecessary hospital admissions through early intervention and community support.
• Advocate for service users and ensure their voices are heard in decision-making.
• Promote anti-discriminatory and anti-oppressive practice.
• Support adults experiencing self-neglect, hoarding behaviours, and complex needs.
• Work with adults with physical disabilities, learning disabilities, mental health needs, and long-term conditions.
• Maintain accurate, timely, and detailed case records.
• Prepare professional reports, assessments, and documentation for panels and meetings.
• Monitor and review outcomes to ensure services remain effective and appropriate.
• Apply relevant legislation including the Care Act 2014, Mental Capacity Act 2005, Human Rights Act 1998, and Equality Act 2010.
• Use professional judgement to manage complex and high-risk situations.
• Build positive working relationships with service users, families, carers, and partner agencies.
• Prioritise workload effectively to meet statutory duties and organisational targets.
• Respond to urgent referrals and crisis situations as required.
• Participate in supervision, reflective practice, and continuing professional development.
• Maintain Social Work England registration and adhere to professional standards.
• Contribute to service improvement initiatives and share best practice within the team.
• Support individuals to achieve positive outcomes and remain safely within their communities.
• Promote strengths-based practice and community-based solutions.
• Ensure all interventions are person-centred, outcome-focused, and legally compliant.
• Work independently and adapt quickly to changing service demands as an agency social worker.
