What you will do:

• Conduct timely Care Act assessments, ensuring that care plans meet the specific needs of adults in the community.
• Perform Mental Capacity Act (MCA) assessments and facilitate Best Interest decisions, always putting patient welfare at the heart of each decision.
• Lead and manage Section 42 enquiries efficiently, safeguarding vulnerable adults and promoting their wellbeing.
• Work collaboratively with health system partners to ensure a seamless integration of services within the hospital setting.
• Maintain and update accurate records, providing clear and concise documentation to support patient care plans.

Qualifications / Requirements / Person Specification:

• Experience working within an acute hospital setting is required.
• Strong understanding of Care Act assessments, MCA assessments, and Section 42 enquiries.
• Ability to work effectively within a complex and busy service environment.
