What you will do

Mental Health Assessments: Conduct initial and comprehensive assessments of health and social care needs, both independently and alongside colleagues.
Care Coordination: Manage care coordination responsibilities for a designated caseload of service users, promoting recovery and social inclusion.
Care Programme Approach (CPA): Act as a Care Coordinator under the CPA framework, ensuring high-quality care planning and delivery.
Multi-Disciplinary Working: Work closely with psychiatrists, social workers, psychologists, occupational therapists, and other professionals to provide integrated care.
Mental Health Support: Deliver person-centred interventions and support individuals experiencing a range of mental health conditions.
Primary Care Liaison: Provide mental health liaison and link-working support with primary care services and partner agencies.
Clinical Leadership: Supervise and support junior staff members, students, and trainees, contributing to workforce development.
Team Participation: Participate in MDT meetings, duty responsibilities, and clinic rotas to ensure effective service delivery.
Risk Management: Undertake risk assessments and implement risk management plans to promote safety and wellbeing.
Record Keeping: Maintain accurate, timely, and professional clinical records in line with NMC standards and organisational requirements.

Qualifications / Requirements

Professional Registration: Current NMC registration as a Registered Mental Health Nurse (RMN).
Experience: Previous experience working within Community Mental Health Teams or Mental Health Services.
Assessment Skills: Strong experience undertaking mental health assessments, care planning, and care coordination.
CPA Knowledge: Good understanding and practical experience of the Care Programme Approach framework.
Risk Management: Ability to assess, manage, and respond effectively to risk and safeguarding concerns.
Communication Skills: Excellent verbal, written, and interpersonal communication abilities.
Leadership Skills: Experience providing supervision, mentoring, or support to colleagues and students.
Teamwork: Ability to work effectively within multidisciplinary and multi-agency environments.
