What you will do

Ward-Based Pharmacy Services: Deliver clinical pharmacy services across acute hospital wards, ensuring safe and effective medicines management.
Medication Screening: Utilise Cerner systems to review prescriptions, screen medication charts, and support patient safety.
Medicines Optimisation: Conduct medication reviews and provide recommendations to improve treatment outcomes.
Multidisciplinary Working: Collaborate closely with doctors, nurses, and allied health professionals to deliver integrated patient care.
Clinical Advice: Provide expert pharmaceutical advice and support to healthcare staff on medication-related matters.
Patient Safety: Identify and resolve medication-related issues, ensuring compliance with best practice and clinical governance standards.
Discharge Planning: Support safe and effective discharge processes through accurate medication reconciliation and counselling.
Documentation: Maintain accurate clinical records and pharmacy documentation in line with departmental procedures.
Professional Practice: Ensure compliance with national guidance, local policies, and professional standards at all times.

Qualifications / Requirements

Pharmacy Qualification: Master's Degree in Pharmacy or equivalent recognised qualification.
Registration: Current registration with the General Pharmaceutical Council (GPhC).
Hospital Experience: Previous experience working within an acute hospital pharmacy environment.
Clinical Knowledge: Strong understanding of medicines optimisation, clinical pharmacy services, and acute care pathways.
System Experience: Experience using Cerner or similar electronic prescribing and clinical information systems is highly desirable.
Communication Skills: Excellent verbal, written, and interpersonal communication abilities.
Teamwork: Ability to work effectively within multidisciplinary healthcare teams.
Driving Requirement: Full UK driving licence and access to a vehicle are essential.
