Children’s Senior Social Worker – Next Steps (Leaving Care / Transitions)
Location: Durham
Pay Rate: £24.95 per hour PAYE | £32.73 per hour Umbrella
Contract: Temporary | Full-Time (37 hours per week)

Seven Resourcing are currently recruiting for an experienced Children’s Senior Social Worker to join a Next Steps Team within a Children in Care service. This is a rewarding opportunity to support young people aged 16–17 as they prepare for adulthood, independence, and transition into adult life.

The role focuses on high-quality pathway planning, purposeful transitions, and building meaningful relationships with young people to ensure they are supported, heard, and empowered throughout their journey. You will work collaboratively with partner agencies to deliver joined-up support across housing, education, employment, and care planning.

Key Responsibilities

  • Manage a caseload of young people aged 16–17 within the leaving care and transitions pathway
  • Lead on pathway planning, ensuring plans are aspirational, clear, and young-person centred
  • Support young people through key transitions, including moves into supported accommodation and adulthood
  • Work closely with residential homes, supported accommodation providers, education services, and housing partners
  • Develop and implement co-produced safety and independence planning
  • Build strong, consistent relationships that promote stability, trust, and engagement
  • Participate in reflective supervision, team development sessions, and quality assurance processes
  • Ensure the voice of the young person is central to all assessments and decision-making
  • Contribute to multi-agency planning and service development initiatives
  • Support timely planning and positive outcomes through structured pathway work

Requirements

  • Qualified Social Worker with current registration with Social Work England
  • Proven experience within Children in Care, Leaving Care, or Transition services
  • Strong understanding of pathway planning and safeguarding adolescents
  • Experience working with 16–17 year olds in statutory social care settings
  • Ability to manage a caseload independently while working within a team
  • Excellent communication, assessment, and relationship-building skills
  • Commitment to strengths-based and young person–centred practice
