Job Title: Occupational Therapist - Community Adults Team

This community-based role will require you to perform home visits and focus on providing essential equipment, adaptations, and facilitating smooth hospital discharges.

What you will do

• Assess the needs of adults with illness or disabilities within the community.
• Determine the nature and extent of required specialist equipment and adaptations.
• Provide informed advice and guidance to service users.
• Complete thorough assessments to ensure smooth and effective hospital discharge.
• Arrange necessary equipment to support safe discharge to home environments.
• Liaise with architects regarding adaptation plans.
• Collaborate effectively with housing, NHS, and voluntary organisations.
• Check completed adaptations meet assessed needs.
• Refer service users to rehabilitation services when necessary.

Requirements

• Diploma or Degree in Occupational Therapy.
• Prior experience working in physical disability Occupational Therapy.
• HCPC registration.
• Strong written and verbal communication skills.
• Self-motivated with the ability to work under pressure.
• Commitment to personal development.
• Car driver with a willingness to travel.

Desirable Requirements

• Experience with Social Services and knowledge of Disabled Facilities Grant (DFG) processes.
• Familiarity with relevant legislation and partner agencies.
• Systematic approach to work.
• Effective team working skills.
• Flexible with work demands.
