What you will do:

• Work in accordance with the Care Act 2014 to assess, review, and reassess complex needs of clients and their carers, providing tailored advice and access to support services that ensure their safety and wellbeing.
• Employ a strengths-based approach to maintain professional relationships, listening to client choices and aspirations to deliver a personalised service.
• Conduct risk assessments and formulate risk management plans that promote independence while ensuring safety.
• Identify and make enquiries regarding potential abuse or neglect, developing safeguarding plans in collaboration with multi-agency teams.
• Liaise with the Adult Social Care Occupational Therapy Team for manual handling assessments and equipment needs, adhering to relevant health and safety legislation.
• Perform specialist functional assessments across daily living activities, from engaging in meaningful activities to exploring adaptive technology.
• Collaborate with stakeholders and voluntary services to foster innovative ways of working and maintain open dialogue.
• Manage time, workload, and priorities effectively, escalating concerns as needed, and maintain accurate records using electronic systems.

Qualifications / Requirements:

• Degree in Occupational Therapy or equivalent recognised qualification.
• Current HCPC registration as an Occupational Therapist.
• Experience working within Mental Health Services, Adult Social Care, or community Occupational Therapy settings.
• Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding legislation.
• Excellent assessment, risk management, and care planning skills.
• Strong communication and relationship-building abilities.
• Full UK driving licence and access to a vehicle are essential.
