What you will do:
- Assess, diagnose, and treat clients with diverse conditions, ensuring the highest level of care within the community.
- Manage complex rehabilitation and falls prevention caseloads, using your expertise to make a real difference.
- Undertake advanced physiotherapy assessments for patients with intricate multi-system conditions.
- Collaborate with a multidisciplinary team to develop evidence-based rehabilitation programmes and care plans.
- Monitor, review, and adjust outcomes to maximise patient independence, supporting discharge planning and care package adjustments.
- Deliver impactful training and education sessions to colleagues, enhancing the team's overall effectiveness.
- Maintain precise, up-to-date records using Azeus Care or other designated systems.
- Engage in supervision, reflective practice, audits, and service development to refine your practice further.
- Communicate complex information clearly and succinctly to patients, carers, and partner agencies to ensure comprehensive understanding.
