Sanctuary Personnel is recruiting for an experienced Senior Practitioner Social Worker to join a dedicated Discharge and Planning Team in Stafford. This excellent locum Social Worker opportunity offers a competitive rate of £33.30 per hour and the chance to support children, young people, and families through effective care planning, transition support, and multi-agency working.

This role is ideal for an experienced Children's Social Worker with strong skills in care planning, discharge planning, safeguarding, and partnership working. As a Senior Practitioner, you will provide expert social work practice, support colleagues, and help ensure children and young people experience safe and successful transitions.

Key Responsibilities:

• Manage a complex caseload involving children and young people requiring discharge planning, transition support, and ongoing social work intervention.
• Complete high-quality assessments, reviews, and care plans to support positive outcomes for children and families.
• Coordinate effective discharge and transition arrangements, ensuring appropriate support is in place.
• Work closely with children, young people, parents, carers, health professionals, education providers, and partner agencies.
• Support children and young people moving between services, placements, or stages of care.
• Identify and manage safeguarding concerns, ensuring risks are assessed and addressed appropriately.
• Provide professional advice, guidance, and mentoring to social workers within the team.
• Prepare detailed reports, assessments, and documentation in line with statutory requirements.
• Ensure case records are accurate, timely, and compliant with Children's Social Care procedures.
• Promote best practice and contribute to improving outcomes for children and families.

Applicant Requirements:

• Qualified Social Worker with a recognised Social Work qualification.
• Registered with Social Work England.
• Significant post-qualified experience within Children's Social Care.
• Previous experience within Discharge Planning, Care Planning, Children in Care, Looked After Children (LAC), Safeguarding, or Child Protection services.
• Strong understanding of statutory social work processes and safeguarding legislation.
• Excellent assessment, communication, organisation, and relationship-building skills.
• Ability to manage complex cases and work effectively within a multi-disciplinary environment.
• Full UK driving licence and access to a vehicle is desirable.
