Social Worker – Adult Services (Hospital Discharge Team)
Location: Newport, South Wales
Pay Rate: £20.98 per hour (PAYE) | £28.01 per hour (Umbrella)
Contract Type: Temporary (24 Weeks)
Start Date: 18th May 2026
Hours: 37.5 hours per week

The Role

Seven Resourcing are currently recruiting for 2 experienced Social Workers to join a Hospital Discharge Team within Adult Services in Newport. This is an excellent opportunity to work within a fast-paced, multi-disciplinary environment supporting individuals to safely return home from hospital.

Key Responsibilities

• Support safe and timely hospital discharges in line with best practice and legislation
• Work collaboratively within a multi-disciplinary team including healthcare professionals and support staff
• Complete assessments of individuals’ needs, including care planning and risk assessments
• Work closely with families and carers to ensure appropriate support is in place
• Promote independence and wellbeing through effective discharge planning
• Coordinate services and resources to prevent delayed transfers of care
• Maintain accurate case records and documentation in line with statutory requirements
• Contribute to improving discharge pathways and overall patient experience

Requirements

• Qualified Social Worker with relevant Social Work degree or equivalent
• Registered with Social Care Wales
• Previous experience within Adult Social Care, ideally within hospital discharge or acute settings
• Strong understanding of safeguarding, legislation, and best practice
• Ability to work under pressure in a fast-paced environment
• Excellent communication and partnership-working skills
