The role offers a blend of home visits and flexible working, with travel required across Cambridgeshire.The initial contract is for 9 weeks, with a strong possibility of extension for longer-term project work.Key Responsibilities Complete reviews of complex and high-cost care packages for adults with Learning Disabilities and Autism.Assess care and support arrangements to ensure positive outcomes are being achieved.Work collaboratively with individuals, families, providers, and partner agencies.Identify opportunities to improve support arrangements while maintaining person-centred practice.Undertake Care Act assessments, reviews, and support planning where required.Work closely with Contracts and Brokerage Teams to ensure appropriate service provision.Maintain accurate case records and complete reports within required timescales.Ensure all interventions are delivered in accordance with relevant legislation, policies, and best practice guidance.Promote independence, wellbeing, choice, and control for service users.
