Role overview
An exciting opportunity has arisen for an experienced Adults Social Worker to join the Learning Disabilities Team in Darlington. This ongoing locum role offers a competitive daily rate of £207.14 and the chance to deliver high-quality, strengths-based social work in line with the Care Act 2014.
You will play a key role in promoting independence, wellbeing, and choice for adults with learning disabilities, working within a supportive and forward-thinking team environment.

What you will do

Undertake Care Act assessments and reviews using a strengths-based approach.

Develop and review person-centred care and support plans.

Promote independence, wellbeing, and informed choice for adults with learning disabilities.

Complete reassessments as individual needs change.

Identify and support unpaid carers, completing Carer’s Assessments where appropriate.

Conduct Section 42 safeguarding enquiries under the Care Act 2014.

Assess and manage risk in accordance with safeguarding principles and legislation.

Undertake Mental Capacity Assessments and facilitate Best Interest decisions.

Work collaboratively with health professionals, housing services, and community organisations.

Support transitions from Children’s Services into Adult Services.

Manage a varied caseload and maintain accurate, timely case records.

Engage in continuous professional development and reflective practice.
