Carry out Care Act 2014 assessments to identify and address service user needs
Undertake Mental Capacity Assessments and support informed decision-making
Make Best Interest decisions in collaboration with families and professionals
Work effectively in a fast-paced environment delivering high-quality support
Collaborate with the Short Term Support Service – Prevention team
Engage with community resources to improve outcomes for service users
