What you will do:
- Manage and organize work assignments, providing comprehensive nursing care and conducting risk assessments within individual care plans.
- Conduct initial and urgent patient assessments with accuracy and efficiency.
- Hold responsibility for the mental health clustering of patients, ensuring information accuracy and timely transfer between teams.
- Lead patient-centered reviews, case conferences, and multi-agency meetings, advocating for the welfare and recovery of service users.
- Work collaboratively within a multi-disciplinary team and engage with other care providers to develop and deliver cohesive care strategies.
- Empower and facilitate service users and their carers in making informed decisions about their care and treatment.
- Support, supervise, and mentor junior staff and student nurses, contributing to their professional development and ensuring a high standard of patient care.
- Maintain accurate documentation and records, crucial for the audit and effectiveness of mental health services.
- Comply with Trust policies and guidelines, adhering to statutory and professional requirements for practice within the Mental Health Act (1983).
