What you will do:

- Assess and treat patients in the emergency department and acute medical wards, covering a wide variety of conditions including frailty, musculoskeletal, and neurological issues.
- Collaborate with a multidisciplinary team to deliver patient-centred care.
- Develop and implement personalised treatment plans to facilitate patient recovery.
- Monitor patient progress and adapt strategies to achieve optimal outcomes.

Requirements:

- HCPC registration is required.
- A valid driver's licence is necessary for traveling between hospital sites and community visits.
- Demonstrable experience in an acute hospital environment, particularly in the emergency department and acute medical wards.
