What you will do:

Manage a specialist caseload of community stroke patients, carrying out comprehensive occupational therapy assessments, and developing individualised rehabilitation plans to improve functional independence and quality of life.

Deliver evidence-based neuro-rehabilitation interventions, such as cognitive, perceptual, upper-limb, and activities of daily living (ADL) rehabilitation, and adapt treatment programmes based on ongoing evaluation of patient progress.

Work as an autonomous practitioner within a multidisciplinary team, collaborating with physiotherapists, speech and language therapists, nurses, and social care professionals to coordinate integrated stroke care in the community.

Assess for and recommend equipment, environmental adaptations, and risk management strategies to enable a safe discharge, maximise independence, and support patients in their own homes.

Provide leadership and supervision to junior staff, assistants, and students, and contribute to service development, clinical governance, audit, and the implementation of evidence-based practice.
Requirements:
