What you will do:

• Manage a specialist caseload of community stroke patients, carrying out comprehensive occupational therapy assessments, and developing individualised rehabilitation plans to improve functional independence and quality of life.

• Deliver evidence-based neuro-rehabilitation interventions, such as cognitive, perceptual, upper-limb, and activities of daily living (ADL) rehabilitation, and adapt treatment programmes based on ongoing evaluation of patient progress.

• Work as an autonomous practitioner within a multidisciplinary team, collaborating with physiotherapists, speech and language therapists, nurses, and social care professionals to coordinate integrated stroke care in the community.

• Assess for and recommend equipment, environmental adaptations, and risk management strategies to enable a safe discharge, maximise independence, and support patients in their own homes.

• Provide leadership and supervision to junior staff, assistants, and students, and contribute to service development, clinical governance, audit, and the implementation of evidence-based practice.

Requirements:
