What you will do:

Assess and implement tailored therapeutic plans for patients within the Intermediate Care setting.

Facilitate timely hospital discharge and assist in the discharge-to-assess process.

Collaborate with multidisciplinary teams to provide holistic patient care.

Utilise your experience with SystmOne, if applicable, for efficient patient management.

Travel to various sites and patients' homes as required, so having a car is essential.
Responsibilities / General Duties:

Bring your intermediate care experience to the role, ideally with hospital discharge or discharge-to-assess experiences.

Utilise SystmOne for record-keeping and patient management, if you have prior experience.

A full driving licence and access to a car are crucial due to the nature of the role requiring travel.
Required:
You must be HCPC registered and possess a driving license with access to a car, as travel to various locations is part of the job.
