What you will do:

• Assess and implement tailored therapeutic plans for patients within the Intermediate Care setting.

• Facilitate timely hospital discharge and assist in the discharge-to-assess process.

• Collaborate with multidisciplinary teams to provide holistic patient care.

• Utilise your experience with SystmOne, if applicable, for efficient patient management.

• Travel to various sites and patients' homes as required, so having a car is essential.

Responsibilities / General Duties:

• Bring your intermediate care experience to the role, ideally with hospital discharge or discharge-to-assess experiences.

• Utilise SystmOne for record-keeping and patient management, if you have prior experience.

• A full driving licence and access to a car are crucial due to the nature of the role requiring travel.

Required:
You must be HCPC registered and possess a driving license with access to a car, as travel to various locations is part of the job.
