What You Will Do

• Conduct thorough assessments of patients who have experienced falls and develop personalised care plans.

• Deliver high-quality physiotherapy services in community-based settings.

• Utilise SystmOne for accurate documentation and effective case management.

• Travel across Hull and the East Riding to provide care at patients’ locations.

• Maintain HCPC standards, ensuring professional and ethical practice.

Requirements

• HCPC registration as a Physiotherapist.

• Experience in community physiotherapy, falls management, and patient assessments.

• Ability to travel locally (UK driving licence required).
