What You Will Do

Conduct comprehensive assessments to develop tailored support plans promoting independence and adult transitions.
Collaborate with families, carers, and adult service teams to ensure seamless transition planning.
Undertake Mental Capacity Assessments for individuals aged 16+ and manage Court of Protection documentation.
Coordinate with a multidisciplinary team, including Enablement, Eligibility, and Occupational Therapy Teams.
Hold regular reviews to assess progress and adjust support plans as needed.
