What you will do:
- Conduct comprehensive mental health assessments and deliver interventions as part of the Home Treatment Team's Gatekeeping service.
- Co-ordinate care plans and implement appropriate support tailored to individual needs.
- Work collaboratively with multidisciplinary teams to ensure patient-centred care.
- Monitor patient progress and adapt care plans as necessary to optimise outcomes.
- Provide emotional support and guidance to both patients and their families.
Requirements of the Registered Mental Health Nurse:
- A degree or equivalent in Mental Health Nursing is essential.
- Demonstrable experience as a Registered Mental Health Nurse is required to succeed in this role.
- Valid NMC Registration is a must to practice as a nurse in this position.
