What You Will Do

• Conduct thorough assessments of adults and elderly residents with physical disabilities, identifying their specific care and rehabilitation needs.

• Develop and implement personalised rehabilitative solutions aimed at enhancing independence and improving the quality of life for individuals.

• Work closely with a multidisciplinary team, assisting with triage and responses to health and social care needs across primary and secondary care settings.

• Provide occupational therapy interventions within patients’ homes and through remote consultations, ensuring a holistic, person-centred approach.

• Maintain accurate records, documenting progress and outcomes of therapy to ensure effective monitoring and review of treatment plans.

Requirements

• Must be a registered HCPC Occupational Therapist.

• A valid driving licence is essential to facilitate home visits within the Bexley area.
