What You Will Do

Conduct thorough assessments of adults and elderly residents with physical disabilities, identifying their specific care and rehabilitation needs.

Develop and implement personalised rehabilitative solutions aimed at enhancing independence and improving the quality of life for individuals.

Work closely with a multidisciplinary team, assisting with triage and responses to health and social care needs across primary and secondary care settings.

Provide occupational therapy interventions within patients’ homes and through remote consultations, ensuring a holistic, person-centred approach.

Maintain accurate records, documenting progress and outcomes of therapy to ensure effective monitoring and review of treatment plans.
