What you will do:

- Undertake functional and holistic occupational therapy assessments in community settings.
- Deliver rehabilitation-focused interventions aimed at promoting independence.
- Support hospital discharge processes and admission avoidance pathways.
- Recommend and prescribe equipment and minor adaptations to facilitate day-to-day activities.
- Complete mental capacity assessments and contribute to best interest decisions.
- Work collaboratively with social workers, health colleagues, reablement teams, and partner agencies.
- Maintain accurate case records in line with council policies and statutory requirements.

Requirements:

- You must have community-based occupational therapy experience with a focus on rehabilitation.
- Proven ability to manage a varied and fast-paced caseload effectively.
- Access to a car and the ability to travel across the locality with confidence.
- Must be registered with the Health and Care Professions Council HCPC.
