What you will do:
- Conduct comprehensive functional assessments with a focus on rehabilitation and restorative approaches.
- Develop and implement personalised reablement plans, including equipment and adaptations as necessary.
- Set clear, achievable goals to promote optimal outcomes for the citizens of Manchester.
- Work collaboratively with Reablement Support Workers to deliver timely and effective interventions.
- Communicate effectively with clients, families, and other professionals to support reablement processes.
