Job Purpose:
• To deliver high-quality occupational therapy assessments and interventions within social services
• To support adults in maximising independence, safety, and wellbeing within the community
• To work collaboratively with multidisciplinary professionals to deliver person-centred care

Key Responsibilities:
• Complete comprehensive occupational therapy assessments for service users
• Deliver targeted interventions to support daily living activities and promote independence
• Develop, implement, and review personalised care and rehabilitation plans
• Work closely with the Social Services Team to ensure coordinated and effective care delivery
• Liaise with healthcare professionals, families, and external agencies to ensure holistic support
• Maintain accurate, timely, and compliant records and documentation

Essential Requirements:
• HCPC registration as an Occupational Therapist
• Experience working within social services or community-based settings
• Strong assessment, intervention, and care-planning skills
• Excellent communication and organisational abilities
• Ability to work independently while contributing effectively to a wider team
