What you will do:
- Conduct comprehensive geriatric assessments as part of the older people’s pathway, identifying early signs of frailty.
- Collaborate with an inter-agency, inter-disciplinary team to provide specialised assessment and care planning for older people with frailty.
- Deliver assessment, clinical care and management plans for both stable and acutely unwell older individuals or those with chronic conditions.
- Facilitate alternatives to hospital admission where clinically appropriate, and support early hospital discharge.
- Work closely with hospital-based teams to assess, plan, and deliver care for vulnerable patients experiencing complex health and social issues.
- Promote and develop community alternatives to hospital admission and provide support for discharge.

Requirements of the Physiotherapist:
- Qualified physiotherapist with a degree or equivalent in Physiotherapy.
- Recent UK experience in specialised physiotherapy is essential.
- Registered with the HCPC as a physiotherapist to work in the UK.
- NHS experience is advantageous but not essential.

This opportunity is ideal for an experienced band 6 AHP clinician (Occupational Therapist or Physiotherapist) looking to gain valuable experience in Acute Frailty Units.
