What You Will Do:

Complete Care Act assessments and Mental Capacity Act assessments in hospital settings.
Fulfil statutory duties to support safe and timely discharges.
Manage a varied and complex caseload within acute hospital environments.
Work closely with multidisciplinary teams to coordinate effective care planning.
Provide high-quality, person-centred social work interventions.
Work both independently and collaboratively to deliver responsive services.
Travel between sites to support regional hospital services where required.

Requirements:

Qualified Social Worker with relevant adult services experience.
Strong background in hospital or discharge-focused social work.
Experience completing Care Act and Mental Capacity Act assessments.
Ability to travel across sites — access to a car required.
