Reablement Assessor – Livingston
Seven Resourcing | Full-Time | 37 Hours Per Week | £42 per hour

Seven Resourcing is seeking an experienced and motivated Reablement Assessor to join a hospital-based reablement service in Scotland. This full-time position (37 hours per week) is available for an immediate start.

We welcome applications from Occupational Therapists, Social Workers, or Physiotherapists with strong experience in reablement, discharge-to-assess (D2A) practice, community rehabilitation, and functional assessment.

Responsibilities

Complete person-centred functional assessments to determine individual needs and goals
Support rapid hospital discharge and safe transition into community settings
Develop and review reablement plans, monitoring outcomes and adjusting support as required
Promote independence, rehabilitation, and recovery in line with strengths-based practice
Work collaboratively across multi-disciplinary teams
Maintain accurate documentation and case records

Essential Requirements

Qualified Occupational Therapist, Social Worker, or Physiotherapist with valid HCPC/SSSC registration
Proven background in reablement, D2A, intermediate care, community rehab, or preventative care services
Strong knowledge of person-centred and strengths-based approaches
Understanding of local authority practice, safeguarding legislation, and adult social care frameworks
Strong assessment, communication, and report-writing skills
Able to work effectively within multi-disciplinary teams
