What you will do:
Provide specialist diabetes care and support to patients and their families across primary, secondary, and community care settings.
Lead and support clinical initiatives to ensure high-quality, evidence-based diabetes care aligned with best practice and current research.
Educate and empower patients and carers, promoting shared decision-making and self-management.
Develop, implement, and evaluate specialist care programmes for patients with complex diabetes needs.
Act as a clinical leader and mentor within the team, promoting professional standards and best practice.
Manage and coordinate resources effectively to maintain a safe and supportive clinical environment.
Lead and contribute to audit, research, and service improvement initiatives to enhance patient safety and care quality.
