What You Will Do

Lead and coordinate Decision Support Tool (DST) assessments to determine eligibility for NHS Continuing Healthcare (CHC) funding.

Manage complex and high-risk cases, including disputes and joint funding arrangements, providing professional and compassionate support throughout the process.

Work closely with care providers and local authority partners to develop appropriate care packages, whether or not individuals meet CHC criteria.

Undertake Section 117 reviews and contribute to service improvement initiatives to ensure high-quality, person-centred service delivery.

Provide professional supervision, guidance, and support to a small team of social care staff, promoting best practice and continuous professional development.

Liaise with a wide range of internal and external stakeholders to implement service strategies aligned with organisational values and objectives.
