What you will do:

- Complete thorough assessments under the Care Act to identify eligibility and support needs for efficient hospital discharge.
- Conduct Continuing Health Care Assessments and Reviews, ensuring all aspects of care are tailored to individual requirements.
- Carry out Mental Capacity assessments in alignment with the Mental Capacity Act 2005.
- Act as a vital link for hospital discharge processes, collaborating with a multidisciplinary team to support seamless patient transitions home.
- Triage and prioritise cases effectively, ensuring timely and safe discharges.
- Engage closely with patients, families, and healthcare professionals to develop and implement care plans that meet specific needs.
